Wholistic Nursing Leadership Course Completion Form

Participants can earn up to 27 contact hours for fully completing the course. Partial contact hours will be given for each section completed. 

This nursing continuing professional development activity was approved by the Oregon Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

Requirements for Successful Completion of this Educational Activity. To obtain a CE certificate for this activity, the learner must complete the following criteria:

  • Attend all training and sign in on the attendance sheet

  • Complete pre-course assessments

  • Complete the post-course evaluation

  • Complete care plans

With completion, the CE certificate will be emailed to you.
